Applicants are required to provide a copy of as many of the following documents as
possible dated at least 90 days prior to the start of the semester for which the student
is applying to change residency status:
- 12 months of bank or credit card statements showing purchases made in the state of
Utah;
- 12 month report from cell phone company showing origination of phone calls and texts;
- A Utah high school transcript issued in the past year confirming attendance at a Utah
high school in the previous 12 months;
- A Utah voter registration;
- A Utah driver license or identification card;
- A Utah vehicle registration;
- Evidence of employment in Utah;
- Proof of payment of Utah resident income tax for the previous year;
- A rental agreement or mortgage document showing the student's name and Utah address
for at least 12 months prior to the first day of class of the semester for which the
student is seeking resident status; and
- Utility bills showing the student's name and Utah address for at least 12 months prior
to the first day of class of the semester for which the student is seeking resident
status.
If you cannot provide a copy of any of these, provide an explanation for why you do
not possess the piece of evidence.
To show twelve months of continuous residence, you will need to provide twelve months
of bank statements.
