Travel Reporting

In an effort to assist our students and employees in staying healthy, the college is asking employees and student to self-report travel outside of Utah before returning to campus. The information gathered in the travel forms will help the college understand where our employees and students have been and evaluate emerging trendlines.

Thank you for helping us track your traveling experience. We are currently tracking travel outside of Utah. This is merely a precaution that allows the college to monitor any potential risks and better assist any impacted members of our campus. This form applies to students and employees who are planning or have returned from personal or business-related trips.

Alert Level 2

Sustained Community-Level Transmission
All Travelers

  • Travelers should avoid contact with sick people and wash their hands often with soap and water for at least 20 seconds. If soap and water are not readily available, use an alcohol-based hand sanitizer that contains at least 60% alcohol.
  • Travelers should monitor their health and limit interactions with others for 14 days after returning from travel.
  • Travelers that are sick with fever, cough, or have trouble breathing should stay home and call ahead before seeking medical care.
  • Older adults and people of any age with serious chronic medical conditions are at increased risk for severe disease and should consider postponing nonessential travel.

Depending on your travel history, you will be asked to stay home for a period of 14 days from the time you left an area with widespread or ongoing community spread.
